Dr. Jacob is an actively practicing Neurology is the Panama City Area, located near Ascension Sacred Heart - Bay Medical Hospital, on the corner of N. Cove Boulevard and E. Business Highway 98. He is experienced with treating and managing patients diagnosed with:

Patients are seen four days a week: Monday, Tuesday, Thursday, and Friday.Our office sees patients for Morning appointments: from 8:30 A.M. to 12:00 P.M. Monday to Friday. Note we are closed on Wednesdays.We ACCEPT the following insurances:

Generally, most PPO plans of other insurances are accepted, if you don't see your plan on this list, please call the office to verify your coverage before scheduling an appointment.We are currently Not in Network with and/or Accepting:
BCBS "Blue Medicare" plans, Humana HMO, Medicaid, Ambetter, Sunshine Health, Worker's Compensation (consultation).Any patients with HMO Policies require a pre-authorization made by your primary physician's office, and it must be noted our ability to provide care will be greatly delayed due to Prior-Auths if you as our patient possess an HMO plan. We are sorry for the inconvenience.Self-pay consultations are $250.00 for the establishing visit, and $150.00 per follow-up appointments. Self-pay EMG/NCV visits are $450.00 per appointment.
Nerve Conduction Study and Electromyography
NCV/EMG Tests are performed three days a week: Monday, Tuesday, and Saturday.Currently the office performs NCV/EMG testing from 4:30 P.M. to 6:30 P.M. on Monday and Tuesday, and from 8:30 A.M. to 12:00 P.M. on Saturdays.
